Job summary

Circadia Health is seeking an on-site Nurse Practitioner to lead direct patient care within our AI-powered platform across Arizona facilities. You will assess residents, initiate plans of care, and collaborate with care teams to integrate predictive insights into bedside practice.

In this role you’ll train staff to use Circadia’s workflows, interpret trend-based data, and document all encounters promptly. The position covers a 30-40 mile radius with mileage coverage per policy and requires

Qualifications

AZ Nurse Practitioner license and ANA/ANCC-appropriate board certification.

Responsibilities

Conduct initial and follow-up NP assessments for flagged residents.
Initiate or adjust plans of care and coordinate with facility providers.
Train staff to integrate Circadia workflows at the point of care.
Interpret trend-based insights to prioritize high-risk patients and timely interventions.
Complete documentation and charts on the same day of each encounter.
Manage high-volume rounds (approx. 30+ visits/day) across designated SNFs.
Provide feedback to product/clinical ops for workflow improvements.
Mentor NP onboarding and collaborate with clinical teams.
Cover sites within a 30–40 mile radius; mileage covered per policy.
