Circadia Health is seeking a Nurse Practitioner to work across skilled nursing and Home Health AI Care programs in Stockton, CA. You will see patients first, act on real-time AI signals, and coach facility teams to intervene earlier to reduce readmissions.

In SNF settings you’ll round on new admits and flagged patients; in home health you’ll extend proactive care directly to patients’ homes, blending care with tech-to-bedside integration.

Qualifications

• Certified and licensed Nurse Practitioner in CA with ANCC or equivalent.
• 2+ years NP experience with complex adult populations; ability to practice independently.
• Willingness to travel locally and work in startup environments.

Responsabilités

• Conduct initial and follow-up assessments for flagged residents and plan care.
• Train staff on Circadia workflows and bedside integration of AI tools.
• Interpret data trends to prioritize high-risk patients and timely interventions.
• Complete documentation within the same day and maintain charts.
• Cover SNF sites and patient homes within a 30–40 mile radius; mileage covered.
