Job summary

Circadia Health in California is seeking a Nurse Practitioner to support SNF and Home Health AI Care programs, seeing patients first, acting on predictive signals in real time, and coaching facility teams to reduce preventable readmissions.

You will round on new admits and facility-flagged patients, extend proactive care into patients’ homes, and integrate care with Circadia’s AI tools through hands-on training, live demos, and shadowed assessments.

Qualifications

CA NP license (ANCC or equivalent) in Adult/Family/Geriatric/Acute.
Eligible for Medicare participation; company bills on your behalf.
2+ years NP experience with complex adult populations; able to practice independently.
Valid CA driver’s license; comfortable traveling locally.
Thrives in a dynamic, evolving startup environment.
English required

Responsibilities

Direct patient care: conduct initial and follow-up assessments; initiate/adjust plans of care; coordinate with providers.
Tech-to-bedside integration: train staff and standardize Circadia into clinical workflows.
Data into action: interpret trend-based insights, prioritize high-risk patients, drive timely interventions.
Close the loop: complete documentation the same day.
High-volume rounding: 25+ visits/day across SNFs when appropriate.
Quality & product feedback: surface insights to improve workflows and integration.
Mentor & collaborate: support NP onboarding, partner with ops/product teams.
Regional flexibility: cover SNF sites and homes within 30–40 mile radius; mileage covered.
