Job summary

• Knowledge of HIPAA and client rights.

Responsibilities

• Enroll clients and provide counseling on program requirements and responsibilities.
• Visit clients’ homes to orient them to Personal Choices program.
• Develop and monitor personal support plans with budgets and contingencies.
• Coordinate with case managers and FMSA to ensure service quality and safety.
• Prepare reports and communicate program updates to leadership.
