Job summary

South-Alabama-Regional-Planning-Commission is seeking a Personal Choices Counselor to enroll clients, counsel, and educate them about rights and responsibilities under the Personal Choices program. The role involves home visits, budgeting, and coordinating with case managers to ensure quality of care and safety.

Responsibilities include developing personal support plans, monitoring budgets and reporting, and participating in community outreach.

Qualifications

• Bachelor’s Degree from an accredited four-year college or university in Social Work, Behavioral Sciences, Psychology, Geriatric Studies, or a related field with a minimum of two years of work experience in home health or Aging programs.
• Knowledge of HIPAA and client rights; ability to protect HIPAA.
• Excellent oral and written communication skills.
• Ability to relate to elderly and their unique problems.
• Ability to operate office equipment and basic budgeting.

Responsibilities

• Receives referrals from Medicaid Waiver team leads or case managers.
• Contacts potential clients to discuss program requirements and plans for in-home enrollment sessions.
• Visits clients' homes to provide orientation and training for Personal Choices program.
• Develops personal support plans addressing health care needs and backup supports.
• Evaluates and approves personal support plans with a spending plan.
• Forwards plans to Program Manager for approval and notifies team on approval.
• Monitors monthly FMSA budget reports for adherence to plans and care quality.
• Communicates with clients to assess self-directed care and ensure essential needs are met.
• Conducts semiannual in-home visits to assess care quality and safety.
• Collaborates with case managers to protect client rights and health.
• Develops organizational knowledge on aging resources; provides training to case managers.
• Assists with community events to promote aging programs; prepares reports.
