Job summary

Senior Friendship Centers seeks a compassionate Case Manager to work with older adults in Sarasota, conducting in-home assessments, developing individualized care plans, and coordinating essential services to help clients stay safe, healthy, and independent. This is a full-time, Monday–Friday role with travel to local areas and occasional remote work from home.

Qualifications

• Requires a bachelor’s degree in Social Work, Sociology, Psychology, Gerontology, Human Services, or related field.
• Three years of human services case management experience preferred.
• Intermediate computer skills including Microsoft 365 and records/billing software.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess needs of older adults referred for program services.
• Perform in-home assessments to evaluate impact of services.
• Develop, implement, and monitor care plans with clients using community resources.
• Complete DOEA annual assessments for each client.
• Meet productivity goals and document client reports and case files.
• Provide after-hours support and participate in trainings and outreach.
