Job summary

Walworth County’s Aging & Disability Resource Center is seeking a dedicated case manager to support elderly and disabled consumers. The role involves coordinating services, assessing needs, and guiding families through eligibility processes while collaborating with providers and community partners.

The position requires a social work degree with relevant experience; a master’s degree is preferred, and trauma-informed practice is essential.

Qualifications

  • Bachelor's degree in social work or related field with at least one year of relevant experience.
  • Master's degree in social work preferred.
  • Experience with aging and disabled populations preferred.
  • Knowledge of Trauma Informed Care Principles essential.

Responsibilities

  • Facilitate consumer access to services and collaborate with contracted providers to monitor purchased services.
  • Assess individuals and families with psychosocial or physical difficulties; develop and monitor service plans.
  • Determine program eligibility by administering mandated screens.
  • Provide short-term case management, including in-home or community visits.
  • Complete annual protective placement reviews for eligible individuals.
  • Report access and abuse neglect as required by state statute and coordinate with APS.
  • Advocate for consumers and educate their support networks to help reach personal goals.
  • Collaborate with team members and other departments to coordinate care.
