Job summary

JASA is seeking a Wellness Specialist for the Living in Place program in Bronx, NY. You will deliver time-limited care management, blend direct clinical work with community outreach, and help seniors access essential resources.

Under supervision of the Program Director and DoHMH funding, you will balance in-home and community-based services with careful case documentation and compliance.

Qualifications

• Bachelor’s degree in Social Work (BSW) or Psychology is required.
• Two years of professional human services experience is required.
• English proficiency; bilingual Spanish preferred.

Responsibilities

• Care coordination and assessment across home, office, and community settings.
• Time-limited interventions following Living in Place program standards.
• Screen for benefits eligibility and advocate for Medicare, Medicaid, and other entitlements.
• Direct-connect clients to transportation, home care, meals, housing and needed services.
• Develop and implement structured outreach plans with clear goals.
• Represent JASA at community events and interagency meetings.
• Complete electronic case records, reports, and statistics per guidelines.
