Job summary

Presbyterian Villages of Michigan is seeking a Service Coordinator to coordinate programs and services that help residents age in place and meet their needs through assessments and resource connections.

You will work with residents, families and community partners, maintain files, ensure HIPAA compliance, and promote wellness programs. A background in social work/psychology/gerontology with 2+ years of experience is preferred.

Qualifications

• Two+ years of social services experience with elderly and/or family populations.
• Knowledge of available supportive services and local resources.
• Able to advocate, organize, problem-solve and deliver results.

Responsibilities

• Coordinate with community organizations to offer services to residents.
• Perform psychosocial assessments with residents and families.
• Maintain resident files and manage supportive services.
• Prepare and submit required reports and documentation (HIPAA compliance).
• Promote resident involvement in programs and coordinate meals-on-wheels/transportation when needed.
• Attend meetings and trainings; follow policies and procedures.
