Job summary

Presbyterian Villages of Michigan seeks a Service Coordinator to coordinate programs and services enabling residents to age in place, assess needs, and connect with resources across physical, mental, and social health.

You will network with community organizations, conduct psychosocial assessments, manage resident files, develop care plans, and report to HUD as required, while ensuring HIPAA compliance and quality service.

Qualifications

• Two or more years of experience in a social service delivery with elderly and/or family population.
• Minimum degree in social work, psychology or gerontology preferred.
• Working knowledge of Windows, Microsoft Office Suite and resident software data systems (AASC online, Care Guide, RealPage or similar related systems, etc)

Responsibilities

• Coordinate programs and services to enable residents to age in place.
• Network with community organizations to offer residents affordable services.
• Perform psychosocial assessments and interviews with residents and families.
• Maintain resident files, develop care plans and monitor supportive services.
• Ensure HIPAA and other regulations are followed and report to HUD as required.
