Job summary

NHE, Inc. in Greenville, South Carolina, seeks a Resident Service Coordinator to educate residents and connect them with federal, state and community referrals to support aging in place.

The role includes non-clinical assessments, coordinating services such as personal care, monitoring progress, developing health programs, and building partnerships with aging agencies to ensure residents receive appropriate support.

Qualifications

• Bachelor’s degree in social service related field or HUD work experience.
• Knowledge of elderly and disability services.
• Knowledge of referral processes.
• 2–3 years of social service delivery experience.
• Training in cultural competency and bilingual skills are assets.

Responsibilities

• Conduct comprehensive non-clinical assessments of residents for wellness and social needs.
• Help residents to identify, access, and coordinate services, including monitoring of services and follow-up communication with providers.
• Monitor receipt and follow-through of services and encourage resident engagement with providers.
• Proactively develop and arrange educational/preventative health programs and services for residents.
• Develop and sustain partnerships with Area Agency on Aging, Aging & Disability Resource Connection and other providers.
