Institute on Aging seeks a Care Manager I to assess psychosocial needs and coordinate services for low‑income older adults. You will plan and monitor the Community Living Plan, coordinate Medicare/Medi‑Cal resources, and support clients in maintaining independence and quality of life.

Responsibilities include conducting assessments, developing care plans, liaising with providers, and documenting progress while ensuring confidentiality and ethical standards.

Qualifikationen

• BA/B.S in Social Work or related field required.
• One year’s experience working with disabled adults and/or frail elderly required.
• Bilingual highly preferred; English plus Cantonese and/or Mandarin and/or Spanish.
• Exceptional communication and presentation skills relating to the frail elderly, functionally impaired adults, their support systems and teams of health professionals.

Aufgaben

• Conducts comprehensive assessments of residents including psychosocial, physical and mental health needs.
• Writes comprehensive assessments and develops the Community Living Plan.
• Identify and arrange for appropriate community services and monitor their delivery.
• Maintain paperwork and charting for continuity of care.
• Educate clients and significant others about resources and train them to provide their own case management.
• Maintain open communication with providers, physicians and social workers.
• Participate in CQI efforts and team meetings.
