Northeast Florida Area Agency on Aging Inc is seeking a dedicated Information and Referral Associate to support older adults, persons with disabilities and caregivers in Jacksonville. This role focuses on providing information and referrals, answering the helpline, and recording client data with confidentiality.

Responsibilities include triaging calls, facilitating referrals to Medicaid, Medicare, DOEA programs, and other resources, scheduling appointments, and advocating for clients.

Formación

• High School Diploma and at least two years’ experience in information and referral services.
• Experience with HIPAA guidelines and confidentiality.
• Ability to use a web-based database system.
• Strong written and verbal communication skills.
• Ability to work in a fast-paced, high-call-volume environment.

Responsabilidades

• Answer the helpline and triage caller needs.
• Provide information and referrals to Medicaid, Medicare, LTC, and DOEA programs.
• Record caller demographics in the database.
• Schedule appointments using Time Tap Scheduler.
• Follow up on referrals and advocate for clients.
• Collaborate with aging network and other agencies.
