Job summary

StarCare Specialty Health System in Lubbock, TX is seeking a full-time Aging and Disability Benefits and Outreach Specialist to help older adults and people with disabilities access public benefits and community resources.

You will assess eligibility, complete applications for Medicare, Medicaid, SNAP, and other programs, coordinate with agencies, and support outreach to strengthen referrals and enrollment for the PACE program.

Qualifications

• Bachelor's degree in a related field (Human Services, Business Administration, Social Services, Healthcare Administration, or related) is required.
• Proficient in Word, Excel, Outlook, and PowerPoint.
• Valid Texas driver’s license and insurability required.

Responsibilities

• Collaborate with state and federal agencies to facilitate timely benefits eligibility determinations.
• Coordinate, track, document, and report eligibility activities with Aging & Disability Services staff and finance personnel.
• Assist individuals with completing applications and accurate documentation for programs (Medicare, Medicaid, SNAP, etc.).
• Support outreach, enrollment, and referral development for the PACE program with Silver Star Marketing & Eligibility Specialist.
• Maintain confidential participant records and ensure compliance with policies and regulations.
• Attend required trainings and maintain credentials in line with organizational requirements.
