Job summary

StarCare Specialty Health System seeks a full-time Aging and Disability Benefits and Outreach Specialist in Lubbock, TX. This role guides older adults and people with disabilities through Medicare, Medicaid, SNAP, and related programs, coordinating eligibility and applications with public agencies.

Responsibilities include financial assessments, eligibility tracking, documentation, and outreach to strengthen referrals for the PACE program. Travel up to 30% and on-site work required.

Qualifications

• Bachelor's degree from an accredited college/university in Human Services, Business Administration, Social Services, Healthcare Administration, or a related field.
• Proficient in Microsoft Office applications (Word, Excel, Outlook, PowerPoint).
• Must have a current Texas driver’s license and be insurable.
• Must pass pre-employment and annual screenings as required by law or policy.

Responsibilities

• Collaborate with state and federal agencies to facilitate benefits eligibility determinations.
• Coordinate, track, document, and report eligibility activities to ensure timely processing.
• Work with representative payees, guardians, and authorized representatives to obtain required information.
• Support enrollment, annual financial assessments, and documentation collection for eligibility.
• Provide exceptional customer service to participants, families, and referral sources.
