In collaboration with the Silver Star Marketing & Eligibility Specialist, this role also supports community outreach, education, enrollment, and engagement initiatives that increase awareness of the Program of All-Inclusive Care for the Elderly (PACE), strengthen referral relationships, and connect eligible individuals with comprehensive services and supports that enhance their ability to age safely and independently within the community.

  • Work with representative payees, guardians, and authorized representatives, as appropriate, to obtain benefit and financial information.
  • Partner with the Census Development Manager to assess and verify the financial eligibility of prospective PACE participants.
  • Participate in intake and financial eligibility meetings with prospective participants, consumers, and family members to review program requirements, eligibility criteria, and enrollment status.
  • Communicate eligibility determinations to applicants and family members and provide guidance regarding next steps in the enrollment process.
  • Conduct timely follow-up activities to ensure all potential funding sources, public benefits, and financial eligibility options have been fully explored.
  • Collaborate with external organizations, including financial institutions, housing authorities, and government agencies, to obtain required verifications, records, and supporting documentation.
  • Contact referred individuals to schedule appointments, assist with completion of applications and agency forms, and respond to questions regarding state and federal benefit programs.
  • Support the Silver Star Marketing & Eligibility Specialist with community outreach, marketing initiatives, participant recruitment efforts, and referral development activities related to the PACE program.
  • Coordinate with Aging & Disability Services team members to ensure the timely completion of annual financial assessments and the collection of required supporting documentation.
  • Attend and participate in annual Community-Based Organization (CBO) training and other required educational programs.
  • Maintain accurate and confidential participant records in accordance with organizational policies, regulatory requirements, and program standards.
  • Provide exceptional customer service to participants, families, community partners, and referral sources while promoting access to benefits and supportive services.
  • Maintain regular and consistent attendance for the assigned work hours.
  • Ensure all required training and credentials are current and compliant with Center, federal, state, and local regulations, and in addition to StarCare policies and procedures.
  • Complete special departmental projects and other responsibilities with similar skill and work conditions as assigned.
