You will conduct assessments, develop service plans, coordinate housing resources, provide skills training, support employment goals, and document in electronic health records, collaborating with a multidisciplinary team to deliver person-centered care.

Qualificações

• Bachelor's degree required in a related field.
• Experience with behavioral health services preferred.
• Knowledge of recovery-oriented practices recommended.

Responsabilidades

• Provide community-based rehabilitation and housing stabilization services.
• Conduct assessments, develop service plans, and document progress.
• Coordinate housing navigation and access to community resources.
• Support budgeting, employment readiness, and independent living skills.
• Collaborate with providers, case managers, landlords, and agencies.
• Maintain compliant documentation and participate in team meetings.
• Deliver person-centered, recovery-oriented services in the community.
