Job summary

The Monroe County Area Agency on Aging is seeking an Aging Care Manager 2 to support older adults with personal, social, and financial needs through direct guidance, assessments, and care planning. You will coordinate with community resources to ensure access to services.

This role emphasizes compassionate communication, home visits or calls, and accurate documentation in SAMS and PIA systems, with typical hours 8:00 AM to 4:30 PM, Monday through Friday, and some telework after a probationary

Qualifications

• Six months as an Aging Care Manager 1 or County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.

Responsibilities

• Consumer Assessment: Gather information about consumers and their situations to identify needs and locate appropriate services
• Care Management: Provide support to consumers and families to help them adjust to personal or social challenges
• Home Contact: Make home visits and phone calls in alignment with established plans and agency policies
• System Documentation: Complete required assessments and maintain accurate records in SAMS and PIA systems
• Benefit Assistance: Help consumers understand and receive benefits and provide information on relevant laws
• Community Coordination: Work with community resources to ensure consumers can access available services

Education

Bachelor's degree in social sciences / behavioral sciences / human services
