Job summary

Commonwealth of Pennsylvania is seeking an Aging Case Aide 2 in Lehigh County to help seniors access services and plan for long-term needs. You will work in a supportive team to empower older adults and families, guiding choices and referrals to community resources.

Qualifications

• One year as an Aging Case Aide 1; or two years of public contact work in human services.
• PA residency required for this position.
• Must pass a background investigation.
• Veterans' preference information available.
• Ability to perform essential job functions.

Responsibilities

• Referral tracking using agency systems to review and status consumer referrals.
• Intake processing by interviewing consumers to identify needs and eligibility; explain programs.
• Provide person-centered counseling to help choose actions and assist with applications.
• Follow up with consumers, supports, and referral sources to ensure needs are met.
• Maintain case records, memos, letters, summaries, and reports with ongoing documentation.
• Engage in community events and training to expand knowledge and service delivery.
