Job summary

Suncrest Hospice, part of the Optum family, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care to patients and families outside regular hours, including nights and weekends. Expect to visit patients in the community as needed; schedule is Friday 4:30pm through Monday 8am.

Responsibilities include triaging needs by phone, conducting initial nursing evaluations within 48 hours of referral, performing afterhours visits (admissions, death visits, recertifications),

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Able to triage patient needs over the phone until a nurse can provide an in-person visit.
• Responds appropriately and urgently to crisis calls from patients and families after hours.
• Makes the initial nursing evaluation within 48 hours of referral as part of a patient visit.
• Competent to perform afterhours visits including admission, death, recertification and routine visits.
• Identifies and reassesses patient/family needs at least every 15 days.
• Documents problems, goals, interventions, and patient/family response to hospice care.
