Job summary

TFI Family Services is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, custodianship, or aging out of care. You will focus on risk management, crisis intervention, and coordinating services during the aftercare period.

The role requires a bachelor’s degree in human services and a valid driver’s license; training is provided.

A Day in the Life
