Job summary

TFI Family Services, Inc. is hiring a full-time Aftercare Case Manager in Kansas to support children and families who have achieved permanency through reintegration, adoption, or guardianship.

You will focus on risk management, provide crisis intervention, coordinate community services, and maintain thorough documentation to ensure stable, lasting outcomes for families. Strong communication and advocacy are essential as you work with state and local agencies.

A Day in the Life

As an Aftercare Case Manager, you'll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You'll:

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.
