Job summary

TFI Family Services in Chanute, Kansas, is seeking an Aftercare Case Manager to support children and families who have achieved permanency. You will focus on risk management, coordinate services, and meet families in person to support reunification.

We require a bachelor’s degree in human services, a valid driver’s license, and a willingness to learn; training is provided. The role offers competitive hourly pay and comprehensive benefits to help families thrive.

A Day in the Life

As An Aftercare Case Manager, You’ll Support Children And Families Who Have Achieved Permanency Through Reintegration, Adoption, Custodianship/guardianship, Or Aging Out Of Care. You’ll

• Provide crisis intervention and work with public and private agencies offering supportive services.

• Meet with families in-person to support reunification and resolve any concerns.
