Job summary

TFI Family Services is hiring a full-time Aftercare Case Manager to support children and families after permanency has been achieved. The role emphasizes risk management, crisis intervention, and coordination with community and educational services to ensure ongoing stability.

You’ll maintain thorough documentation, advocate for families, and meet with them in person to support reunification and address concerns, while training provided to broaden skills and impact.

A Day in the Life

As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll:

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.
