Job summary

TFI Family Services is hiring a full-time Aftercare Case Manager to support children and families achieving permanency. You will focus on risk management, crisis intervention, and coordinating services with public and private agencies.

You will meet with families to support reunification and document outcomes accurately. The role requires a bachelor’s degree in human services, a valid driver’s license, and a willingness to learn through provided training.

A Day in the Life

As An Aftercare Case Manager, You’ll Support Children And Families Who Have Achieved Permanency Through Reintegration, Adoption, Custodianship/guardianship, Or Aging Out Of Care. You’ll

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.
