Job summary

TFI Family Services, Inc. is seeking a dedicated Aftercare Case Manager in Independence, KS to support children and families after permanency. This full-time role emphasizes crisis intervention, risk management, and coordination with community services to ensure stable reunification where possible.

You will complete documentation, meet families in person, and collaborate with public and private agencies, while training is provided to grow your skills in this important field.

Qualifications

A Day in the Life

As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll:

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.
