Job summary

TFI Family Services, Inc. is hiring a full-time Aftercare Case Manager in Kansas. You will support children and families who achieved permanency, focusing on risk management, crisis intervention, and coordination of services. You’ll document thoroughly and meet families to support reunification.

Required: bachelor’s in human services and a valid driver’s license; we provide training to help you grow and thrive in a compassionate, mission-driven team.

A Day in the Life

As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll:

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.
