Job summary

TFI Family Services in Chanute, Kansas is hiring a full-time Aftercare Case Manager to join our team. You’ll support children and families who have achieved permanency through reintegration, adoption, guardianship, or aging out of care, with a focus on risk management and stability.

Requirements include a bachelor’s degree in human services, a valid driver’s license, and a willingness to learn—training is provided.

• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve concerns.
