Job summary

VNA Home Health & Hospice is seeking an After-Hours Visiting Registered Nurse to provide home health and hospice care during after-hours coverage. Responsibilities include visits, admissions, and hospice death visits, with the ability to work independently and document promptly.

The role requires RI licensure, CPR certification, and strong communication skills. A Bachelor’s degree is preferred, with hospice and OASIS certs preferred but not required and may be obtained during employment.

Qualifications

• RN with RI licensure.
• CPR certification is required.
• Bachelor’s Degree in a relevant discipline is preferred.
• Hospice and OASIS certification are preferred but not required; may be obtained during employment.
• Minimum of one year progressive clinical experience; hospice experience preferred.

Responsibilities

• Provide scheduled after-hours visits, admissions, and hospice death visits.
• Communicate concerns to leadership and follow agency policies.
• Work independently with sound clinical judgment and timely documentation.
• Coordinate with interdisciplinary team and on-call leadership.
