Job summary

EverHeart Hospice in Ohio is seeking an After-Hours Nurse to provide direct patient care after business hours, focusing on symptom management and family education in home and care settings. Responsibilities include skilled nursing, end-of-life care, DME coordination, and seamless patient transitions; you will collaborate with physicians and ensure timely chart reviews.

Requires Ohio LPN/RN license, at least 2 years LPN experience, valid driver’s license, strong communication, and time

Qualifications

• Must hold a current Ohio LPN or RN license.
• Minimum 2 years LPN experience (Med/Surg, Oncology, or Community Health preferred; hospice a plus).
• Valid driver’s license and reliable transportation.
• Strong communication and time-management skills.
• Ability to work independently.

Responsibilities

• Provide skilled nursing care after hours including symptom management.
• Conduct home visits and chart reviews to monitor patient status.
• Educate patients and families on hospice services and insurance benefits.
• Collaborate with physicians and Medical Directors; enter orders as needed.
• Arrange DME and coordinate transitions between home and inpatient care.
• Maintain documentation per agency and regulatory standards.
• Maintain clinical competency and participate in weekend rotations as assigned.
