Pay ranges from $72,000 to $77,000 annually DOE. Schedule is 7 on 7 off, overnight 5pm-8am, with competitive benefits and supportive leadership in a patient-centered environment.

Qualifikationen

• Current Oregon LPN license required.
• Graduate of an accredited nursing program.
• Current CPR certification.
• 1–2 years of acute care nursing experience.
• Home Health or Hospice experience preferred.

Aufgaben

• Respond to after-hours patient calls and triage needs.
• Provide skilled nursing visits in patient homes.
• Complete admissions and follow-up visits as needed.
• Administer medications and treatments as ordered.
• Assess patient conditions and communicate changes to providers.
• Educate patients and caregivers on care plans and symptom management.
• Maintain accurate and timely documentation.
• Collaborate with physicians, case managers, and interdisciplinary teams.
