Albany Home Health and Hospice is seeking a compassionate LPN for the On-Call/After Hours Team in Albany, OR. The role focuses on skilled nursing visits in patients' homes, after-hours triage, and admissions with a strong emphasis on patient education and coordination with physicians and care teams.

Requirements include an active Oregon LPN license, CPR certification, and 1–2 years of acute care nursing experience; Home Health or Hospice experience is preferred.

• Licensed Practical Nurse (LPN) with current Oregon license.
• CPR certification current.
• Graduate of an accredited nursing program.
