Job summary

Signature Healthcare at Home is seeking an on-call/after-hours Licensed Practical Nurse (LPN) to join our Home Health/Hospice team in Oregon. The role focuses on triage, patient visits, and coordination with the interdisciplinary group to deliver compassionate care outside regular business hours.

You’ll work 7 days on/7 days off, ensure timely documentation, administer medications, educate families, and support palliative and rehabilitative goals in patients’ homes.

Qualifications

• Graduate of an accredited school of nursing (Required).
• 1-2 years of acute care in an institutional setting (Preferred).
• 1-2 years of hospice or home health experience (Preferred).
• Licensed Practical Nurse in Oregon (Required).
• CPR certification (Required).
• Effective written and oral communication (Required).
• Strong observation, math skills, and nursing competency.
• Self-directed with minimal supervision.
• Valid driver with insured vehicle.

Responsibilities

• Provides nursing care within the nursing process and LPN scope.
• Assesses patient/family needs and revises care plans with RN and physician.
• Initiates preventive rehabilitative and palliative care; administers meds as prescribed.
• Involves patient/family to meet goals and delivers care instructions.
• Documents services timely and accurately per standards.
• Collaborates with family, hospice team, and home health team.
• Attends staff and interdisciplinary meetings; supports Signature Healthcare at Home mission.
• Arranges equipment and services as needed.
