Signature Healthcare at Home is seeking an LPN for after-hours, on-call nursing in the Albany area. The role delivers skilled nursing visits in patients’ homes, handles after-hours calls, admissions, and follow-up visits, administering medications and therapies as ordered.

A valid Oregon LPN license and CPR are required. 1–2 years of acute care experience is expected, with home health or hospice exposure preferred.

Qualifications

• Current Oregon LPN License.
• Graduate of an accredited nursing program.
• Current CPR certification.
• 1–2 years of acute care nursing experience.
• Home Health or Hospice experience preferred.
• Valid driver’s license with reliable transportation and auto insurance.
• Strong communication, assessment, and independent decision-making skills.

Responsabilités

• Respond to after-hours patient calls and triage needs.
• Provide skilled nursing visits in patient homes.
• Complete admissions and follow-up visits as needed.
• Administer medications and treatments as ordered.
• Assess patient conditions and communicate changes to providers.
