Job summary

Signature Hospice in Oregon seeks a licensed practical nurse (LPN) to join our on-call home health team. This role works outside regular business hours, providing triage, patient visits, and follow-up in collaboration with the interdisciplinary group.

You will administer medications, document care, and support patients and families toward comfort and independence. A strong clinical foundation, Oregon LPN license, CPR, and a reliable vehicle are required; hospice/home health experience preferred.

Qualifications

• Graduate of an accredited nursing program.
• 1–2 years acute care preferred; hospice or home health experience a plus.
• Current Oregon LPN license; CPR certification required.
• Must have a valid driver’s license and a reliable vehicle.

Responsibilities

• Provide on-call triage nursing during after-hours shifts.
• Complete patient visits and follow-up as needed.
• Collaborate with the interdisciplinary group to meet patient needs.
• Document services and status timely and accurately; maintain care plans.
