Job summary

The Community Hospice in Albany and surrounding counties is seeking a compassionate LPN to join our Hospice Home Care team. This full-time, day-shift role offers Monday–Friday 8a–4p with no weekends or on-call and competitive pay of $26.50–$36.90 hourly, based on experience and education.

Provide skilled nursing at patients' homes, coordinate with the interdisciplinary team, and support families through education and symptom management.

Qualifications

• Current New York State LPN license.
• 1 year LPN experience.
• 1–2 years in home care and/or hospice preferred.
• Valid driver's license and reliable transportation.
• Strong communication, documentation, and organizational skills.
• No hospice experience? Willing to learn.

Responsibilities

• Provide skilled nursing care to hospice patients in the home setting.
• Monitor patient status and communicate changes to the interdisciplinary team.
• Educate, support families, manage symptoms with compassion.
• Assist with implementation of the hospice plan of care.
• Collaborate with nurses, physicians, social workers, chaplains, and aides to ensure quality care.
