Job summary

Heartland Hospice in Dearborn, MO is seeking an After Hours RN to provide compassionate in-home care during evenings, nights, weekends, and holidays. You will respond to urgent needs, coordinate with the daytime team, and ensure continuity of care for patients and families.

This on-call role requires an active MO RN license, CPR certification, and at least 1 year of nursing experience. You’ll work closely with LPNs, CNAs, and physicians to implement the Plan of Care and support bereavement

Qualifications

• Active RN license in Missouri or eligibility to obtain
• Minimum 1 year nursing experience
• CPR certification required
• Compassionate, calm and dependable in urgent care situations
• Familiarity with electronic medical records (EMR) systems

Responsibilities

• Respond to patient needs after normal business hours
• Serve as liaison between patients, families, physicians, and hospice team
• Document visits and communications accurately
• Assess patient and family needs and guide clinical decisions
• Collaborate with interdisciplinary team (LPNs/LVNs, CNAs)
• Maintain Plan of Care and ensure continuity of care across shifts
• Participate in quality assessment and performance improvement programs
• Work independently while coordinating with daytime team
