Job summary

Elara Caring in Bingham Farms, MI is seeking an After-Hours/On Call Registered Nurse to join a dedicated hospice team. This full-time role operates on a 7-on/7-off schedule, delivering compassionate care to patients and families after hours.

You will assess needs, educate families, and collaborate with physicians and the care team to uphold the interdisciplinary Plan of Care. A valid RN license and home health experience are preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
