Job summary

Elara Caring is seeking an After Hours/ On Call Registered Nurse to join our elite team serving patients in their homes. This salaried role travels up to 50% and supports hospice care across diverse settings, delivering compassionate, high‑quality care in the patient’s living environment.

You will report to the Clinical Team Manager and contribute to an interdisciplinary plan of care, while ensuring compliance with safety and quality standards. A nursing degree and RN license are required.

Qualifications

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse (RN).
• 1 year of experience in a clinical care setting.

Responsibilities

• Ensure all activities align with Elara Caring’s vision and Hospice team leadership.
• Assess hospice patients and families to identify needs documented in records and reports.
• Assess patient needs across physical, psychological, social, and spiritual factors, collecting data for care plans.
• Communicate significant findings to Clinical Supervisor, physician, facility, and other care personnel.
• Counsel and involve patient, facility, and family in the Interdisciplinary Plan of Care.
• Teach hospice philosophy and pain/symptom management to staff in other settings.
• Provide support at time of death and perform bereavement assessments.
