Graham Healthcare Group is seeking an After Hours Registered Nurse (RN) to provide compassionate hospice care in patients' homes across the Erie, PA area. You will assess, manage symptoms, and support families during evenings, nights, weekends, and on-call shifts.

This role requires an active RN license in the state, about two years of RN experience (Hospice preferred), and a valid driver's license, with travel to patient locations ~20% of day. A $10,000 first-year bonus is offered.

Responsabilidades

• Deliver compassionate, patient-centered hospice care with an interdisciplinary team.
• Respond to after-hours needs with urgent assessments and symptom management.
• Support patients and families during evenings, nights, weekends, and on-call shifts.
• Maintain accurate EMR documentation with strong clinical judgment.
• Work independently, but collaborate as part of the care team.
• Travel to patient homes within assigned territory.
