Qualified candidates have an active RN license and at least two years of RN experience, preferably in hospice, plus a valid driver’s license for approximately 20% travel.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Respond to after-hours and on-call needs with urgent assessments and symptom management.
• Support families during evenings, nights, weekends, and crisis situations.
• Apply clinical judgment to coordinate care and maintain EMR documentation.
• Work independently in patient homes and travel within assigned territory.
