Job summary

CorsoCare Hospice is seeking an After Hours Registered Nurse to provide essential hospice care in patients' homes during evening, overnight and weekend shifts in Oakland & Macomb Counties.

You will be supported by IntellaTriage to triage calls and will coordinate with the IDG to ensure safe, compassionate, and compliant care.

Licensed in Michigan with RN experience and the ability to work independently are required.

Qualifications

• Graduate from an accredited school of nursing.
• Licensed to practice as an RN in Michigan.
• Minimum 2 years RN experience, preferred.
• Active patient contact within past 3 years, preferred.
• Ability to work independently and make quick judgments.
• Ability to supervise others appropriately.
• Ability to respond to crises outside of hospital settings.
• Respect for diverse cultural and religious needs.

Responsibilities

• Regularly assesses and reassesses patient needs in the home.
• Provides Hospice nursing services, treatments and care planning.
• Initiates procedures per patient’s Hospice plan and safety.
• Teaches and counsels patient and family; coordinates with IDG.
• Supervises other nursing staff and ensures continuity of care.
• Maintains records and complies with HIPAA and agency policies.
