Job summary

CorsoCare Hospice is seeking an On-Call After Hours Registered Nurse to provide essential hospice nursing care for terminally ill patients in home settings, during evenings, overnights, and weekends in Oakland & Macomb Counties. You will support patients and families with empathy, guide care plans, and collaborate with an interdisciplinary team.

Responsibilities

• Regularly assesses and reassesses nursing needs of Hospice patients in the home.
• Provides dietary counseling and Hospice nursing services.
• Initiates nursing procedures for Hospice care and safety.
• Observes signs and reports changes to physician/IDG.
• Educates and counsels patient/family on home care.
• Supervises and trains other nursing staff.
• Develops and re-evaluates patient/family care plans with IDG.
• Performs nursing procedures as ordered by doctor.
• Keeps abreast of business updates and maintains records as required.
• Follows Hospice policies and HIPAA regulations.
• Communicates with On-Call Clinical Manager when unable to meet needs.
• Provides after-hours coverage and flexes schedule as needed.
• Works with After Hours Team and interdisciplinary group in patient care.
• Coordinates plan of care for patients in SNF/NF/ICF/MR.
• Organizes schedule to attend required meetings.
• Complies with infection control policies.

Position Summary:

The After Hours Registered Nurse (RN) provides essential nursing care for terminally ill Hospice patients during evening, overnight and weekend shifts. Responsibilities include supporting patients and their families in home care settings, offering guidance and empathy during difficult times, and providing quality care to our patients within their home.

Required Experience for After Hours Registered Nurse:

• Graduate from an accredited school of nursing
• Currently licensed as a registered nurse to practice in the state.
• Minimum 2 years of experience as a registered nurse, preferred.
• Active patient contact within past three years, preferred.
• Ability to work independently, make accurate, and at times, quick judgments.
• Ability to supervise others appropriately.
• Ability to respond appropriately to crisis outside of a hospital setting.
• Acceptance of and adaptability to different social, racial, cultural and religious modes.

Accountability for After Hours Registered Nurse:

• Regularly assesses and reassesses the nursing needs of the Hospice patients in the patient’s home as needed.
• Provides dietary counseling.
• Provides Hospice nursing services, treatments and preventive procedures.
• Initiates nursing procedures appropriate for the patient’s Hospice care and safety.
• Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition.
• Teaches, supervises, and counsels the Hospice patient and family members about providing care for the patient.
• Supervises and trains other nursing service personnel.
• Develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
• Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders.
• Keeps abreast of business updates by actively participating in company meetings or reviewing recordings, ensuring up-to-date knowledge of organizational developments.
• Maintains records as required by Hospice.
• Follows the policies and procedures of Hospice. Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
• Always communicates to the On-Call Clinical Manager if unable to meet a patient’s need or perform a procedure.
• After-hours nurses will be required to provide coverage as needed, demonstrating flexibility and readiness to support patients during critical times.
• Work collaboratively with After Hours Team to ensure patient needs are met.
• Works with interdisciplinary group concept of patient care.
• Coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR.
• Organizes work schedule and utilizes time management to be able to attend all required meetings.
• Complies with agency infection control policies and protocols.
• Assist with orientation, teaching and training as requested.
• Other duties as assigned by Director of Clinical Services or Clinical Manager.

This position entails standing for long periods of time. While performing the duties of this job, there will be considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must have a current valid driver’s license, auto liability insurance and reliable transportation as travel may be required.

The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. Occasional travel to different locations may be required.
