Job summary

Snowline Health is seeking a dedicated Registered Nurse for after-hours, on-call coverage to provide compassionate, comprehensive care to patients and families in the Diamond Springs area. The role requires full-time availability with core hours from 4:00pm to 12:00am, three days on and three days off.

You will collaborate with the Medical Director and on-call leadership to coordinate hospice and palliative care.

Qualifications

• Graduation from a Baccalaureate Degree Nursing Program, Associate Degree Nursing Program or a diploma in Nursing from an accredited school of nursing
• Registered Nurse with current licensure to practice professional nursing in the State of California
• Minimum of one year experience required; hospice is preferred

Responsibilities

• Provide compassionate nursing care for patients and families
• Coordinate with physicians, administrators, and other team members
• Document patient visits, orders, and care plans in EMR
• Perform after-hours nursing visits as needed within the program
• Maintain compliance with regulatory standards and company policies
