You will conduct urgent home visits, administer meds per care plans, and collaborate with the interdisciplinary team to adjust care as needed, delivering compassionate end-of-life support. This role requires strong clinical judgment, independence, and reliable transportation.

Competenze

• Active RN license in Illinois.
• Hospice, pallat ive care, or home health nursing experience preferred.
• Strong clinical assessment and critical thinking.
• Ability to work independently and manage on-call responsibilities.
• Excellent communication with patients, families and care teams.

Mansioni

• Respond to after-hours calls with clinical assessment and guidance.
• Conduct urgent home visits to evaluate and manage symptoms.
