Job summary

Advocate Aurora Health is seeking a Hospice Registered Nurse to provide compassionate, patient- and family-centered care in Milwaukee area. You will conduct assessments, triage, and interventions for hospice patients, including after-hours shifts and in-home visits as clinically indicated.

Responsibilities include symptom management, patient education, and coordination with physicians and the interdisciplinary team, with thorough documentation in the EMR and a commitment to quality and safety.

Qualifications

• RN license in the state of Wisconsin or applicable state.
• BLS certification and current clinical competencies.
• At least one year of hospice/home health nursing preferred.

Responsibilities

• Incorporate clinical decision-making and critical thinking into hospice nursing.
• Assess, triage, and intervene for hospice patients after hours.
• Respond to calls, educate caregivers, and manage symptoms per goals of care.
• Conduct in-person visits to relieve symptoms and ensure comfort.
• Document encounters accurately in the EMR and coordinate care.
