Job summary

Advocate Health in Illinois seeks an experienced hospice registered nurse to deliver compassionate, patient-centered care. You will perform comprehensive assessments, triage after hours, and coordinate with physicians and the care team to align with hospice goals.

This role requires strong clinical judgment, excellent communication, and travel to patient homes in a home/community setting. An active RN license and BLS are required.

Qualifications

• Completion of an accredited or approved nursing program.
• Active RN license in the state of practice.
• Valid driver’s license.
• BLS certification – American Heart Association.
• Typically one year of nursing experience in any clinical setting.
• Proficiency with electronic medical records and computers.
• Excellent communication and interpersonal skills.

Responsibilities

• Incorporate responsibility, accountability, critical thinking, and sound clinical decision-making into daily nursing practice.
• Provide comprehensive nursing assessment, triage, and intervention for hospice patients during after-hours shifts.
• Respond to patient and caregiver calls to assess symptoms, educate, and determine interventions aligned with hospice goals.
• Conduct in-person visits to manage acute symptom changes, provide crisis support, and ensure comfort and safety.
• Manage complex end-of-life symptoms including pain, dyspnea, agitation, anxiety, nausea, and secretions.
• Initiate and carry out physician orders and standing hospice protocols within scope of practice.
• Provide emotional support and education to patients and caregivers, including guidance on treatment decisions.
• Collaborate with physicians, APPs, and interdisciplinary teams to coordinate care.
• Document all encounters and communications accurately in the EMR with clear handoffs.
• Adhere to hospice policies, regulatory requirements, and professional nursing standards.
