Job summary

VIA Health Partners in South Carolina is seeking an experienced Registered Nurse for end-of-life care. You will perform nursing assessments, visit patients, and implement and adjust plans of care to support patients and their families.

The role emphasizes compassionate, patient-centered care, emergency triage by telephone, and collaboration with an interdisciplinary team to manage complex needs. RN license and CPR are required.

Qualifications

• Diploma or associates degree in nursing is required (BSN preferred).
• Current RN license; multistate NLC preferred.
• Minimum 2 years related experience in direct patient care.
• Experience in hospice or palliative care desirable; pediatric experience for Kids Path Nurse a plus.
• CPR certification required; CHPN preferred.

Responsibilities

• Coordinate patient/caregiver care and conduct nursing assessments.
• Provide interventions and visits per plan of care; adjust as needed.
• Emergency triage by telephone and in home visits.
• Maintain liaison with healthcare providers for multidisciplinary care.
• Maintain clinical records and documentation per guidelines.
