Qualifications

• Diploma or Associate degree in nursing required; Bachelor’s preferred.
• Active, unrestricted RN license required; CPR certification.
• CHPN preferred; nursing experience in hospice/palliative care.
• Computer proficiency required.

Responsibilities

• Coordinate patient/caregiver care and manage nursing assessments.
• Make visits and follow the plan of care; revise as goals change.
• Provide education to patients and caregivers; ensure safety and quality of care.
• Triage emergencies by telephone and through home visits; collaborate with interdisciplinary team.
