Job summary

VIA Health Partners in Albemarle, NC, is seeking a compassionate RN to coordinate and deliver hospice care to patients and families, focusing on comfort, education, and quality of life.

The role emphasizes nursing assessments, care plan development, emergency triage, and collaboration with the interdisciplinary team to meet patient goals; strong communication and computer skills are required, with CHPN encouraged.

Qualifications

• Diploma or Associate Degree in nursing; Bachelor's Degree preferred.
• Current, active RN license (multistate) required.
• Current CPR certification; CHPN preferred.
• Minimum 2 years related nursing experience in life-limiting illnesses.
• Experience with pediatric population preferred (Kids Path) and CPR for adult/child.
• Computer proficiency required.

Responsibilities

• Coordinate patient/caregiver care; conduct nursing assessments.
• Make visits to provide interventions and revise plans of care as needed.
• Develop, implement, evaluate plans of care including education.
• Emergency triage by telephone and home visits.
• Maintain liaison with healthcare providers for multidisciplinary care.
• Document clinical information and maintain records; complete documentation.
• Maintain medication profile; provide teaching on meds.
• Adhere to policies, standing orders and guidelines.
